COMPLIMENTARY SEMI-PERMANENT EYEBROWS
For Chemotherapy Patients

Conditions

The Client should provide a medical clearance from the Attending Physician to proceed with the permanent makeup procedure. Enhance! Dermagraphics Studio Inc. ( EDSI ) should be furnished with a copy.

• The client should sign the Release Forms and the Medical Disclosure Forms.
• An allergy test is required prior to the procedure.
• The client agrees to travel to the Richmond Studio for the procedures.
• The client agrees to be photographed (Before & After shots) for reference, case portfolio, and other professional /educational purposes as may be required by EDSI.
• A donation is requested but not required. All donations will be sent to the Poor Sisters of St. Claire Convent in Mission, B.C. along with your prayer petitions (if applicable).
